    わが国の社会状況の変化:超高齢化、女性の社会進出、晩婚化、晩産化等に伴い、子世代、特に嫁が行ってきた高齢者介護は、実の息子・娘による介護、老老介護、ヤングケアアラー、ダブルケアなど多様化しており、伴う問題やその解決策、支援策も異なる可能性がある。これまでの我われの研究で、女性家族介護者の幸福感が低いこと、40代介護者の社会的ケア関連QOLが低いことを明らかにしており、この年代に特有の介護、育児、責任ある仕事等の重複プレッシャーやストレスの影響が考えられた。そこで、40・50代中年介護者にフォーカスをした介護のみならず、育児、仕事での支援も含め、ポジティブ心理学を応用した、介護者の幸福感を高める総合支援プログラムの開発・評価を行うことを目的としている。 本年度は、40・50代の介護者、特にダブルケアと呼ばれる介護と育児の両方を担っている介護者の実態把握、既存介護者支援プログラムについて、認知症の特徴やその接し方について、またポジティブ心理学に基づく育児支援、効果的な接し方、さらに幸福感を高める仕事法支援等の文献整理を行った。 また、介護者調査を用いて、当該年代の介護者を、介護のみ、介護と育児、介護と仕事、介護と育児と仕事を担うの4グループに分けて、幸福感やQOL等の実態を把握する分析を行った。

    本研究が対象とする課題は、介護現場にテクノロジーをいかに導入するかということに加えて、テクノロジーの導入によって介護の効率、介護に対する考え方、介護を支える社会保障制度に対する信頼がいかに変わるかを、介護にかかわる介護職、家族介護者、高齢者、これからの高齢者である中高年、そしてテクノロジーの開発者の視点から明らかにし、技術や介護情報、テクノロジーを導入することによって福祉そのものに対する人々の信頼をいかに維持・醸成するか?という問いに答えようとするものである。 本年度は、地域在住の中高年を対象に行った介護におけるテクノロジー導入に対する意識調査データを解析し、高齢期のADLやIADLに関連したテクノロジー導入へのアクセプタンスを評価するとともに、介護におけるテクノロジー導入希望に対する関連要因には性差があること、学歴や介護提供経験が関連すること、また各々のテクノロジーに対する考え方に差があることが明らかになっている。また、介護における機器や技術開発者へのインタビュー調査からは、開発者が技術開発のためだけではなく、導入や機器使用継続に向けて介護現場との関係性の強化などを含めた様々な取り組みを行っている実態が明らかになった。 2025年問題を目前に、介護保険制度の継続においては経済的・資源的に極めて緊迫した課題を抱えており、高齢者本人の自律と尊厳のためにも、介護とテクノロジー融合が介護システムの持続性の中に位置づけられることが期待される。介護施設における導入だけでなく、地域在住の一人暮らし等高齢者におけるテクノロジー導入の可能性について、今後の調査・解析によって検討をしていく予定である。

    The purpose of this research is to study sustainability of a long-term care system by examining the roles of family, public Long-term care insurance, and community. We analyzed national data in Japan and the US to examine how family caregiving trends have changed, examined the relationships between the usage of in-home service and institutionalization among older adults with long-term care needs, and conducted a web-based questionnaire study among male working caregivers. This study shows the diversification of long-term care situation in Japan, which requires modification of public Long-term care insurance services. Also, the study suggests that companies in communities may be one of possibilities to sustain long-term care system.

By focusing on not only family caregivers but also post caregivers, who experienced providing care to family, as well as middle-aged population, who has not experienced caregiving yet, we delineated needs of comprehensive support system among community-dwelling older adults with long-term care needs and the families. The study also revealed a new feature of caregiving burden, which family caregivers were dealing in their daily caregiving situations.

    The current project examined social trends in caregiving in Japan by using national data of Comprehensive Survey of Living Conditions over the past 20 years, including changes since the implementation of the LTCI program. I used macro and micro level analysis from this dataset to predict future possible family caregiving profiles, which told whom and what types of caregiving should be given priority in establishing future policy at the societal level. The other project was a collaborative research at Johns Hopkins University to compare caregiving situations in Japan and the US. It aimed to clarify how care tasks were shared in both countries, and the impact these tasks had on family relationships and the psychological burdens. This comparison study had helped to reconsider the balance between formal and informal care.

    This study is aimed at implementing the PDCA cycle (process improvement technique consisting of 4 steps:Plan,Do,Check, and Act) which has two main components, medical, long term care, and welfare care fieldsites and the university. We have planned to cooperate with the medical, long term care, welfare fieldsites by examining together the Tsukuba-city medical welfare case examination meetings, and providing feedback to Tsukuba City by analyzing their needs survey. We performed a scientific study using several types of data analysis, including big data from national long-term care benefit expenditures data and from the Comprehensive Survey of Living Conditions. We examined the regional comprehensive care promotion policy , including these findings, to clarify how to advance the cooperation between medical and long-term care, what kind of support require the persons in need of care, children with disabilities and the use situation of nursing care insurance services.

    Some medical associations in Japan have issued end-of-life care guidelines including the Japan Geriatrics Society and the Japanese Society for Dialysis Therapy. We have created two notebook-type tools for elderly patients and their families to support their decision-making process when they need to consider AHN (artificial hydration and nutrition) or dialysis therapy. The tools include information related to AHN or dialysis therapy as well as those related to withholding and withdrawal of AHN or dialysis therapy. The tools guide patients and their families to write down the patients values and beliefs together with their narrative information so that they make a decision based on the narrative. We aim to promote shared decision-making among patients and their families and physicians and care providers.

    従来の研究では、介護負担感の関連要因として、高齢者の認知機能や日常生活動作能力(Activity of Daily Living)の低下、認知症による問題行動といった要介護者の要因に加え、介護者の年齢や、経済状態が低いことが報告されているが、これらの要因への介入は難しいために、負担軽減のための十分な支援に繋がっていないのが現状である。本研究では、直接の支援につなげることが可能な要因の一つとして地域の支え合いやネットワークといった地域環境要因に注目し、これらが介護者の精神健康に与える影響について検討を行うことを目的とした。本研究結果から、地域支援のあり方について提言を行うことができると期待される。 2010年3月~6月、A県の協力を得て、県内すべての15介護保険者と1介護保険広域連合の要介護認定者リストから、要介護1~5の認定を受けている第1号被保険者のうち、無作為に抽出された5639名を対象に、郵送による自記式質問紙調査を実施した。調査内容は、介護の負担、抑うつの程度、地域環境要因としての介護のしやすさ、地域のネットワーク、さらに、介護状況、介護保険サービスの利用状況等とした。回収率は51.4%で、最終分析対象は2511票であった。平均年齢は63歳、女性介護者75.4%、4割を超える介護者が65歳以上という老老介護の現状にあること、4割近くの介護者が「仕事」と「家族の介護」の両方を担っていること、認知症状を有する要介護者の介護者は7割近くいることが明らかになった。加えて、介護者自身に健康不安を抱えているものも4割近くおり、非常に脆弱な介護者の状況が明らかになった。また、地域環境要因と精神健康との関連では、地域環境要因の指標の一つである地域に対する信頼と互酬性を持つ介護者の方が、介護の負担や抑うつの程度は低く、地域の環境が介護者の精神健康に影響していることが明らかになった。今後は、本結果を基に地域支援に向けた具体的施策についての検討を行う予定である。
