    介護施設において認知症の人の意思を尊重したケアを実践するためには、介護職・看護職がその人の認知機能に合わせて意思を引き出す「関係を前提とした自律（relational autonomy）」の支援が必要である。そこで、介護職が日常ケアの中から、認知症の人の行動から「意思」を推察しようとしているか、しているとしたら、どのような場面でどのように対応しているのか把握する調査を実施した。全国の認知症対応型共同生活介護と地域密着型介護老人福祉施設から4000か所（抽出率24.3％）を抽出し、事業所長宛に調査票を郵送、介護職リーダーに回答を依頼し、webでデータ収集した。所属の倫理委員会承認後に実施した。調査方法は、一般的に介護職が対応を困難と感じる２つの場面（①入浴したがらない、②就寝後部屋から出て歩く場面）に関するビネットを提示し、各場面で認知症の人の思いを探る対応をするかしないか、する場合にはどのように対応するか自由記述を求めた。分析は、ケア提供者の言動に分節化してコード化し、ケア提供者の意図を解釈してカテゴリー化した。その結果、「安心感のある関係づくり」がケアの前提として必要とされ、入浴や就寝とは関係ない話題や行動で過ごしながら、思いに近づく情報を収集していた。その方法は、本人に直接理由を聞く「主観的世界の探索」と、記録などから探る「客観的情報からの推測」があった。入浴や就寝まで段階や時間をかけて、ケアを受ける意思がみられるまで「本人が受け入れやすい提案をし続けて待つ」一方で、ケア提供者の「意図通りにならないことを許容」する記述も多くみられた。ケア提供者は、認知症の人との安心感のある関係を前提として、探索、推測、提案して待つなど、本人の意思に近づく様々な方略があることを示した。この内容は、日本認知症ケア学会第24回大会で発表する予定である。

    本研究は、地域在住高齢者における「主観的ウェルビーイング」の維持・向上を目指すアクションリサーチである。アクションの企画は、地域住民や自治体職員、研究者、その他専門職とともにアクション内容を検討し、研究者と自治体職員が主となってアクションを決定・実行する予定である。 研究初年度の令和3年度は、初めに研究対象地域候補であった自治体に研究協力の依頼をし、研究フィールドが決定した。次に、実行可能なアクションの企画・決定に際する材料（基礎資料）を準備するため、主観的ウェルビーイングの先行研究を収集し、さらに当該地域において過去に実施した調査データの再分析を行った。再分析の結果は資料としてまとめ、自治体職員に説明・共有した。しかしながら、新型コロナウィルス感染症の蔓延防止に伴う様々な状況により、実行可能なアクションを検討する会議の開催には至らなかった。 なお、既存の調査データの再分析は、当該地域において実施した郵送調査において、地域で展開されている健康関連事業（通いの場への参加等）に無関心だと想定された高齢者を対象に、2018年度に実施したインタビュー調査のデータである。分析結果は、地域で展開される健康関連事業に非参加または無関心な層に効果的にアプローチできるヒントが詰まったものであり、本研究のアクションを考えていく上で活用できる貴重な資料である。 本年度、検討会は開催できなかったが、既存の調査データの再分析によって抽出されたニーズの一部に即した内容のリーフレットと、「高齢期の豊かな心・ウェルビーイング」の普及・啓発を目的として出版されているリーフレットを配布した。リーフレットは、新型コロナウィルスのワクチン接種会場等、高齢者が集まる機会・場で配布した。また、広報にフレイル予防の情報を定期的に掲載することも決定し、その中で主観的ウェルビーイングの維持・向上に繋がる内容も発信していく予定である。

    本研究は、高齢者の意思決定に影響する要因として日本人の価値観、とりわけ「自立」意識に着目し、意思決定プロセスにおける自立意識の役割を検証することを目的とした。今年度は、高齢者の意思決定に影響する要因について文献調査を行った。とりわけ、高齢者の「迷惑をかけたくない」という思いに着目し、この思いの背景や思いに包まれた意識や価値観についての文献レビューを行い、論文にまとめた。この文献調査結果を基に、70歳以上の高齢者を対象としたインタビュー調査実施に向け、調査項目の内容を検討している。

    本研究が対象とする課題は、介護現場にテクノロジーをいかに導入するかということに加えて、テクノロジーの導入によって介護の効率、介護に対する考え方、介護を支える社会保障制度に対する信頼がいかに変わるかを、介護にかかわる介護職、家族介護者、高齢者、これからの高齢者である中高年、そしてテクノロジーの開発者の視点から明らかにし、技術や介護情報、テクノロジーを導入することによって福祉そのものに対する人々の信頼をいかに維持・醸成するか？という問いに答えようとするものである。 本年度は、地域在住の中高年を対象に行った介護におけるテクノロジー導入に対する意識調査データを解析し、高齢期のADLやIADLに関連したテクノロジー導入へのアクセプタンスを評価するとともに、介護におけるテクノロジー導入希望に対する関連要因には性差があること、学歴や介護提供経験が関連すること、また各々のテクノロジーに対する考え方に差があることが明らかになっている。また、介護における機器や技術開発者へのインタビュー調査からは、開発者が技術開発のためだけではなく、導入や機器使用継続に向けて介護現場との関係性の強化などを含めた様々な取り組みを行っている実態が明らかになった。 ２０２５年問題を目前に、介護保険制度の継続においては経済的・資源的に極めて緊迫した課題を抱えており、高齢者本人の自律と尊厳のためにも、介護とテクノロジー融合が介護システムの持続性の中に位置づけられることが期待される。介護施設における導入だけでなく、地域在住の一人暮らし等高齢者におけるテクノロジー導入の可能性について、今後の調査・解析によって検討をしていく予定である。

    Using the framework of the socioemotional selectivity theory, which explains the association between the perception of remaining time in life (future time perspective) and emotional well-being, this study aimed to identify pragmatic approaches for promoting the emotional well-being of older adults. A targeted intervention was administered involving weekly, 30-minute yoga sessions designed specifically for older adults aged 70 and older who were experiencing functional declines. The findings suggest that this yoga practice intervention might potentially influence the emotional well-being and future time perspective of older adults.

    本研究の目的は地域高齢者のスピリチュアリティに焦点をあてた主体的な健康生活のための具体的支援の方向性と実践の手がかりを得ることである。 平成29年度はA市介護福祉課等の協力機関と連絡調整を行った。さらに、前年度にA市S地区高齢者を対象に、三澤等が作成したスピリチュアリティ評定尺度を用いた健康生活に関する調査結果（1,557名）の分析を行った。高齢者の健康はウェルビーイングおよび自己実現を目指すことから、スピリチュアリティを焦点とした健康づくりの意義が確認された。 平成30年1月～3月の予備調査を経て、平成30年6月～8月に高齢者の健康づくりプログラムの開発に取り組んだ。目標は高齢者の主体的な健康への意欲と共通意識が高まることである。プログラムはスピリチュアリティ尺度の構造を基盤として、今を生きる自分を中心とした「他者とのつながり」、「乗り越えてきた道のり」の要素から、「笑いヨガ」と「グループ回想法」を取り入れた。笑いヨガはストレスの軽減がなされ、回想法は自分を安定させ、心の拠り所になると仮定した。A市S地区の高齢者を対象に、公募により24名を選定して、介入を6回実施した。介入前後のアンケート調査、毎回実施前後の血圧・唾液アミラーゼ測定を行った。同時期に同市の高齢者を対象に対照群調査（49名）を実施した。介入前後では検定により、スピリチュアリティ評定尺度得点は有意に高く、精神的健康度が高くうつ状態が軽減し、生活満足度が高くなることが確認された。介入群と対照群との比較では尺度得点、精神的健康度、生活満足度に介入群に有意な高値が認められた。実施したプログラムは都市近郊の一地域の高齢者を対象にしたプログラムであるが、地域高齢者の健康づくりとして有効である可能性が示唆されたといえよう。平成31年2月に対象地区の高齢者に「健康づくりプログラムの開発と評価」について発表会を実施した。
